The Social Worker (SW) functions in an expanded Social Work role demonstrating a professional autonomy and expert skill to participate in caring for patients with 22q11.2 deletion syndrome (22q11.2DS) as part of a team. The principal responsibilities of the SW position in the Hearts and Minds clinic will include: playing a leadership role in the advancement of Social Work practice and standards for patients with 22q11.2DS and their families and caregivers; emphasizing patient functioning and quality of liFe in the community; implementation of best practices in the care of patients; performing quality improvement and administrative responsibilities; performing research activities; delivering, coordinating and participating in education - in coordination with educators within UHN and beyond; supporting the education of SW, medical, nursing and allied health professionals and trainees. - [color=rgb(51, 51, 153)]Completion of a Masters degree in Social Work or recognized equivalent[/color]
- [color=rgb(51, 51, 153)]Membership in good standing with the Ontario College of Social Workers and Social Service Workers

- [color=rgb(51, 51, 153)]Eligibility for academic appointment at a University
[/color]
- [color=rgb(51, 51, 153)]Prior experience working with patients and families with developmental disabilities, dual diagnoses and/or serious mental illness is essential
[/color]
- [color=rgb(51, 51, 153)]Relevant experience working with patients and families with 22q11.2DS, congenital and/or other multi-systemic disorders is an asset
[/color]
- [color=rgb(51, 51, 153)]Relevant experience within the fields of social work, psychiatry (including the forensic system) and other areas of medicine such as neurology, cardiology and/or genetics

- [color=rgb(51, 51, 153)]Experience in identifying community-based resources for adult patients with learning disabilities and/or psychiatric illnesses
[/color]
- [color=rgb(51, 51, 153)]Experience in the transition of patients and families from the paediatric to the adult healthcare setting

- [color=rgb(51, 51, 153)]Experience in family and genetic counselling or relevant background
[/color]
- [color=rgb(51, 51, 153)]Experience in program development for patients and families would be an asset
