That train of thought or joke was started by Joan Rivers. She was trying to remain current when joking about Mrs. Obama. Since the first lady was so popular Joan Rivers thought that referring to Mrs. Obama as a tall man trying to pass as a woman would make a funny joke. It was a silly reference to Michelle Obama's height.
